Are 16bit images with colormaps actually common? I have never supported them in my software and haven't encountered any in my various projects. Who has run into them? Is this an area we should be worried about handling efficiently?

FWIW, I've never seen one.

There is now a colormapped image with 16 bits per sample and 65536 colors available at:

If you download this file, then you can no longer say that you have never seen one. :-)

Doh! There goes my defence. For what it is worth my GDAL access library worked fine for the 16bit colormap but my viewer (OpenEV) only works on 256 color colormaps.

Also, the 12bit file (not surprisingly) completely flusters GDAL.
